Abstract

A 47-year-old Malay woman was admitted in January, 2002, complaining of dyspnoea and cough with non-purulent sputum for 1 week and fever over the past 2 days. She had type 2 diabetes, dyslipidaemia and hypertension, had been diagnosed with asthma by her primary physician as she had suffered from episodes of wheezing since June, 2000. She had never smoked. Spirometry done at the time showed a restrictive pattern (which was attributed to obesity) with FEV1 (forced expiratory volume in 1 s) of 0·65 L (27% of predicted) and FVC (forced vital capacity) of 0·76 L (26% of predicted).
